Exercise can play a valuable role, even when dramatic weight loss is not the immediate outcome.
Fresh Start for Fatty Liver & Metabolic Health is a 12-week structured exercise programme designed to improve fitness, strength and metabolic health through progressive, appropriately challenging training.
Who is the programme for?
The programme is designed for adults aged approximately 40–75 with fatty liver disease, elevated liver enzymes or central adiposity who want a more focused and closely supervised approach to exercise.
It may suit you if:
You have been advised to become more active following blood tests or a diagnosis
You want to improve metabolic health rather than focusing solely on the number on the scales
You want structured guidance around higher-intensity exercise
You would like objective measurements of your progress
You want to build a more sustainable long-term exercise routine
Building your baseline
Your initial assessment considers the broader picture, including:
Medical history, medications, and alcohol intake
Liver enzymes and metabolic markers where available
Waist circumference and waist-to-hip ratio
Body composition
Strength and functional capacity
Aerobic capacity through submaximal or VOâ‚‚ max testing
This gives us a baseline from which your training can be appropriately planned and progressed.
Tracking meaningful change
Across the programme we can monitor:
Liver and metabolic markers where available
Waist circumference and waist-to-hip ratio
Body composition and central adiposity
Strength
Aerobic capacity
Energy and recovery
At week 12, relevant assessments are repeated to help identify measurable progress and inform the next phase of your training.
Your 12-week training programme
The programme uses a combination of interval, circuit and resistance exercise.
Sessions typically involve 30–45 minutes of structured activity, with intensity progressing as your fitness and tolerance improve.
Higher and lower-intensity intervals can be combined with resistance exercises targeting major muscle groups, creating an efficient way to build both cardiovascular and muscular fitness. Strength work can either be integrated into circuit sessions or completed separately two to three times per week. Most importantly, progression is based on how you are actually responding. Exercise volume and intensity are increased gradually, taking symptoms and recovery into account.
Challenging does not mean reckless
Higher-intensity training is not appropriate for everyone at every stage.
The supplied pathway therefore includes referral for medical or specialist review where advanced liver disease is suspected, liver enzymes are unexplained or rapidly worsening, significant symptoms such as jaundice or abdominal pain develop, or there is another contraindication to higher-intensity exercise.
Fresh Start is about applying the right challenge, not simply doing more.

Aerobic and higher-intensity interval exercise can improve important metabolic risk factors and support reductions in liver fat, even without large changes in total body weight.
That makes exercise about much more than simply ‘burning calories’.
